Anyone have a relatively painless way to remove ceramic tile from a concrete slab? Once removed, what's the best way to level the slab/remaining thinset for new flooring installation? Thanks.

Go to the rental place. They make a wonderful tool that will pop the tiles off the floor very painlessly. This tool will also scrape off all of the old mastic fairly well too.

From there, it's a floor scraper and a lot of elbow grease. Or, you can use a floor sander made to handle concrete.

Next, use a leveling compound over the ENTIRE floor surface. Then you'll be ready for your new floor.
